Summary
Pharmacists have a long-standing history of providing management of anticoagulant therapy. The content of this program will provide the pharmacist with a review of thromboembolic risk factors and coagulation disorders. Utilizing current guidelines and recommendations, this program will provide the learner with the tools necessary to provide routine management of anticoagulant therapy in outpatient settings. Key concepts will be reinforced though case applications.

Objectives
At the completion of this activity, the participant will be able to:
- Discuss treatment strategies for venous thromboembolism and atrial fibrillation
- Discuss oral anticoagulants including indications, efficacy and safety, monitoring, dosing and drug interactions
- Develop individualized plan for peri-procedural management of anticoagulation
Program Overview
Lecture 1: Review of Venous Thromboembolism, Atrial Fibrillation, and other Clotting Risk Factors
In this lecture, the area of focus will be on:
- Discuss risk factors, presentation, and locations of VTE
- Discuss diagnostic methods of VTE
- Recommend an anticoagulation strategy for VTE and atrial fibrillation
- Identify risk factors for complications of atrial fibrillation
Lecture 2: A Quick Review of Using Warfarin
In this lecture, the area of focus will be on:
- Discuss indications and monitoring for warfarin related to diagnosis
- Review dosing, drug and food interactions
- Discuss the options for monitoring INR
- Recognize approved protocols for switching between warfarin and DOACs
Lecture 3: Direct Oral Anticoagulants in the Ambulatory Clinic
In this lecture, the area of focus will be on:
- Review the various agents available, indications, and monitoring for the various DOACs approved for use in the US
- Discuss clinical pearls of use in patients with comorbid disease states
- Recognize approved protocols for switching between DOACs and Warfarin
Lecture 4: Periprocedural management
In this lecture, the area of focus will be on:
- Discuss guidelines and recent literature regarding the periprocedural management of anticoagulation
- Evaluate the risk of bleeding and the risk of thromboembolism in the periprocedural setting for a patient taking anticoagulation
- Develop an individualized plan for the management of a patient’s anticoagulation around the time of a procedure.
- Provide patient education with the goal to optimize therapy and reduce the risk of adverse events
Lecture 5: Cases
In this lecture, the area of focus will be on:
- Apply knowledge and principles taught in this program to patient cases in order to develop treatment plans
Requirements for Successful Course Completion
In order to earn credit for this course, participants must pass a final assessment with a score of 70% or higher. Two chances to pass the final assessment will be given. If a passing score is not earned by the second attempt, the participant must re-enroll in the course. Upon successful completion of all coursework, participants must complete a course evaluation form.
