Summary
Pharmacists have a long-standing history of providing management of anticoagulant therapy. The content of this program will provide the pharmacist with a review of thromboembolic risk factors and coagulation disorders. Utilizing current guidelines and recommendations, this program will provide the learner with the tools necessary to provide routine management of anticoagulant therapy in outpatient settings. Key concepts will be reinforced though case applications.
This course is approved by the Florida Board of Pharmacy for 3.0 contact hours to meet requirements pursuant to CHAPTER 64B16-26, Florida Statutes, and Rule 64B16-26.300, Florida Administrative Code.

Objectives
At the completion of this activity, the participant will be able to:
- Discuss treatment strategies for venous thromboembolism and atrial fibrillation.
- Discuss oral anticoagulants including indications, efficacy and safety, monitoring, dosing and drug interactions.
- Develop individualized plan for peri-procedural management of anticoagulation.
Requirements for Successful Course Completion
In order to earn credit for this course, participants must pass a final assessment with a score of 70% or higher. Two chances to pass the final assessment will be given. If a passing score is not earned by the second attempt, the participant must re-enroll in the course. Upon successful completion of all coursework, participants must complete a course evaluation form.
Continuing Pharmacy Education Requirements
Pharmacy continuing education credit will be earned based on completion of this activity. To receive continuing education credit, your completion must be verified, and a program evaluation must be completed and submitted at the end of the program. Credit will be issued through the CPE Monitor and CE Broker websites.
Florida consultant pharmacist (or Florida pharmacist) numbers, NABP e-Profile ID numbers and participant’s Month and Day of Birth are required for credit issuance. UF CPE is not responsible for unissued education credits where the required information is either omitted or submitted incorrectly. UF CPE will contact registrants once by email to attempt to obtain missing or invalid information.
